Overall Responsibility
Included, but not limited to are the following activities: facilitating linkage to care, adherence counselling and support, contact screening, retention in care and accessing new treatment regimens. The enrolled nurses will be based at various Primary Health Care Facilities and Hospitals within the subdistricts.
- Strengthen and support active TB case finding.
- Work closely with facilities to establish community – based structures that will create demand for HIV/TB screening, care and treatment.
- Coordinate activities with health facilities to ensure that clients referred from the community receive the continuum of care.
- Support the community mobilization activities, including mobile outreach activities for HIV/TB prevention, care and treatment services.
- Employ appropriate messaging to trigger demand-driven TB case finding and provision of treatment of Latent TB Infection (LTBI).
- Screen and collect sputum for diagnostic purposes.
- Facilitate the Up-down referral and Linkage of TB patients between community and facilities.
- Ensure TB patients on treatments attend appointments.
- Report to facility Manager on patients screened, diagnosed, linked to care and visited.
- Update and record patient’s information in standardized registers.
- Ability to screen all clients for TB
- Collect on the spot sputum from all eligible clients
- Generates daily, weekly, and monthly reporting of data for reporting
- Update client records with laboratory sputum results and treatment start date
- Work with the facility-based personnel, e.g. ENAs, Data Capturer to ensure that data is captured on the system and verified to ensure data quality
- Follow up on clients referred to the facility to ensure a continuum of care.
- Liaise with the outreach teams for the follow-up of clients
- Conduct community education on TB prevention, symptoms, and adherence to treatment
- Link with community team on tracing of household contacts of all TB index cases.
- Perform other duties as assigned by the IHPS senior management team
- Supervise ENAs and TB Support Officers as required
- Support data collection and data management activities
- Collect on the spot sputum from all eligible clients
- Generates daily, weekly, and monthly reporting of data for reporting
- Conduct community education on TB prevention, symptoms, and adherence to treatment
Qualifications:
- Matric plus Two-year training as Enrolled Nurse and current registration with the South African Nursing Council (SANC)
- Minimum of two years’ practical experience working in HAST program
- Proven experience, at least two years in the treatment and management DR-TB care
- Minimum of two years’ practical experience in nursing, post registration
- Current Registration with SANC as an enrolled nurse
- 2 years of working experience within a similar environment or any clinical setting
- Knowledge of basic TB, and HIV management
- Experience in health promotion and health education
- Computer literacy in Microsoft Package
- Problem-solving and the ability to use initiative in interpreting data
- Good communication, interpersonal, conflict management and negotiations skills.
