Overall Responsibility
Under the supervision of the Community Programme Coordinator, the Social Worker will provide comprehensive psychosocial support, case management, and rights-based interventions to AYP aged 15-24 within the “My Journey” AYP Programme, with a focus on trauma counselling, GBV response, child protection, and facilitating access to holistic care services under the clinical and programme framework.
Key Performance Areas:
Programme Support:
- Conduct psychosocial assessments and develop individualized service plans for AYP enrolled in the programme.
- Provide trauma-informed counselling and emotional support to AYP, including survivors of GBV, abuse, and human rights violations.
- Facilitate therapeutic group sessions on topics including mental health, positive parenting, gender norms, and resilience building.
- Implement the AYP programme within community safe spaces and through outreach activities.
- Provide crisis intervention and immediate support for AYP in distress or emergency situations.
- Collaborate with the clinical team to ensure integrated psychosocial and biomedical care for clients.
- Make and follow up on referrals for clinical services (HIV testing, PrEP, ART), social grants, identity documents, legal aid, and shelters.
- Facilitate gender-based violence (GBV) prevention programmes and support post-violence care.
- Maintain confidential and accurate client case files, ensuring timely data entry into the programme’s case management system.
- Mobilise communities and raise awareness on child protection, SRHR, and anti-stigma messaging.
- Participate in multidisciplinary team meetings with clinical, M&E, and community staff to ensure coordinated service delivery.
- Support the linkage of AYP to paralegal services and human rights support as needed.
- Compile and submit weekly and monthly case reports and activity summaries to the Clinical Manager.
- Attend regular supervision, case conferences, and continuing professional development sessions.
- Perform any other duties as assigned by IHPS senior management
Qualifications
- Bachelor’s degree in Social Work,
- At least 3-5 years’ experience in case work, group work, and community work,
- Registration with the South African Council for Social Services Profession (SACSSP)
- Experience working with Youth

- Excellent written and verbal communication skills
- Presentation and report writing skills
- Data interpretation and analysis
- Computer literacy – intermediate level in MS Office programmes
- Facilitation skills for coaching, training, and development.
- Problem-solving skills and results driven
- Decision making ability
- Good interpersonal skills
- Excellent verbal and written communication skills in English. Command of other official local languages (multilingual) would be an added advantage
- Valid code driver’s license.
