Social Worker – MSM & TG

The Institute of Health Programs and Systems (IHPS) is a South African non-profit company (NPC) born out of the PEPFAR CDC Track 1 transition of the ICAP, Columbia University, Multi-country AIDS Program (MCAP) in South Africa in 2010. The IHPS mission is “to strengthen health and development systems through program implementation and partnerships”, with a vision to achieve “optimal and sustainable health and development outcomes”. Currently, IHPS is a Global Fund (GF) Subrecipient (SR) under the Aurum Institute, tasked to implement the “Comprehensive HIV Prevention Programmes for Men who have Sex with Men (MSM) and Transgender People (TG) and their sexual partners across all five districts in Limpopo Province viz. Capricorn, Mopani, Sekhukhune, Vhembe and Waterberg from 01 December 2025 to 31 March 2028. As such, IHPS seeks to recruit the positions outlined below, based in Capricorn District, Limpopo Province.

Overall Responsibility

The incumbent will play a central role in providing psychosocial support, case management, and community outreach to key populations, specifically Men who have Sex with Men (MSM), and Transgender (TG) individuals. The position ensures that KP communities receive stigma-free, rights-based, gender-sensitive health and social services, while also contributing to advocacy and empowerment initiatives.

Key Performance Areas:

  • Provide individual or group counselling to MSM & TGP presenting with psychosocial needs and the rescreening.
  • Participate in a multi-disciplinary team to ensure that MSM/TGP are provided with comprehensive psychosocial support services.
  • Provide adherence support services to MSM & TGP who are on Antiretroviral Therapy (ART) and on pre-exposure prophylaxis (PrEP)
  • Facilitate disclosure counselling to MSM & TGP and their caregivers to minimize defaulting
  • Conduct home visits to MSM & TGP with psychosocial needs to determine underlying factors and intervene accordingly
  • Manage cohort groups of MSM & TGP with similar issues and interests for purposes of retention in the Program
  • Provide support to Peer Educators and Peer navigators where necessary
  • Facilitate referrals of layered services where appropriate viz. Health Facilities, Gender Based Violence (GBV) organizations, Home Affairs, Department of Social Development, SAPS and other stakeholders
  • Compile case reports, process notes and referral reports for interventions provided to MSM/TGP.
  • Develop weekly plans and compile activity reports and monthly tracking tool.
  • Assist the Monitoring and Evaluation team during onsite data verification visits
  • Develop monthly and quarterly success stories with support from Supervisor
  • Representing IHPS in all relevant platforms as and when required
  • Attend to program meetings as required
  • Participate in any research related activities as per need.
  • Perform any other duties assigned by the line manager.

Qualifications 

  • Bachelor’s degree in social work
  • Proof of registration with the South African Council for Social Services Profession (SACSSP)

Experience 

  • At least 5 years’ experience in public health sector as a Social Worker, case work, group work and community work
  • Experience in administration and management.
  • Experience working with Key Populations in the HIV/TB Sector

Job requirements

  • Proficient computer skills with Microsoft Word, Excel, PowerPoint
  • SA Citizen or valid work permit to work in South Africa
  • Minimal/Moderate overnight travel (up to 30%) by land or air if required.
  • Willing to work overtime.

Competencies

  • Excellent verbal and written communication skills in English. Command of other official local languages (multilingual) would be an added advantage.
  • Sound problem solving and decision-making skills
  • Effective time management, organising and coordination skills.

Travel Requirements
Local travel is inherent to the position.
